Overview

The combination of a neoantigen vaccine and an anti-PD-1 antibody is an investigational cancer immunotherapy regimen designed to stimulate a patient-specific immune response against unique tumor mutations while simultaneously overcoming immune checkpoint-mediated suppression. A specific implementation of this approach is **SarcVac**, a personalized peptide-based vaccine developed by Sun Yat-sen University. SarcVac is derived from tumor-specific mutation sequences identified through whole-exome and transcriptome sequencing of an individual patient's tumor tissue. When administered alongside an anti-PD-1 monoclonal antibody, the vaccine primes T-cells to recognize neoantigens, while the antibody blocks the PD-1 receptor to prevent the tumor from deactivating these immune cells. This combination is currently being evaluated in Phase I clinical trials for the treatment of advanced or unresectable bone and soft tissue sarcomas, including osteosarcoma, in patients who have failed standard first-line therapies.
