Neomycin sulfate + bacitracin zinc + hydrocortisone + polymyxin b is a topical ophthalmic combination drug composed of three antibacterial agents and one corticosteroid, historically marketed as **Cortisporin** ophthalmic ointment and also available as generic ophthalmic ointment products. The antibiotics provide broad coverage against common ocular bacterial pathogens: neomycin impairs bacterial protein synthesis by binding the 30S ribosomal subunit, polymyxin B disrupts the outer membrane of susceptible Gram-negative bacteria, and bacitracin inhibits bacterial cell wall synthesis; hydrocortisone suppresses corticosteroid-responsive ocular inflammation. The combination is used for steroid-responsive inflammatory ocular conditions when bacterial infection is present or there is a meaningful risk of infection, including inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe, as well as chronic anterior uveitis and corneal injury from chemical, radiation, or thermal burns or foreign-body penetration.
