**Neomycin sulfate + polymyxin B sulfate + bacitracin zinc + hydrocortisone** is a fixed-dose ophthalmic anti-infective and corticosteroid combination marketed as **Cortisporin Ophthalmic Ointment** and available in generic ophthalmic-ointment formulations. Hydrocortisone activates the glucocorticoid receptor to suppress ocular inflammation, while neomycin inhibits bacterial protein synthesis through the 30S ribosomal subunit, polymyxin B disrupts Gram-negative bacterial outer membranes by binding lipopolysaccharide, and bacitracin inhibits bacterial cell-wall synthesis by blocking undecaprenyl-pyrophosphate recycling. It is used for corticosteroid-responsive inflammatory conditions of the conjunctiva, cornea, and anterior ocular segment when bacterial infection is present or likely, including chronic anterior uveitis and corneal injury from chemical, thermal, or radiation burns or foreign bodies.
