**Neratinib + rifampin** is a combination of two drugs: neratinib, an oral irreversible pan-HER tyrosine kinase inhibitor, and rifampin, a widely used antibiotic and potent CYP3A4 inducer. Neratinib inhibits epidermal growth factor receptor (EGFR), human epidermal growth factor receptor 2 (HER2), and human epidermal growth factor receptor 4 (HER4), thereby blocking downstream signaling involved in cell growth and proliferation, primarily used in the treatment of HER2-positive breast cancer[1][5]. Rifampin is used for tuberculosis and other bacterial infections due to its ability to inhibit bacterial DNA-dependent RNA polymerase[4]. In combination, rifampin greatly reduces plasma neratinib levels due to CYP3A4 induction and is contraindicated except for specific drug interaction research purposes. The combination is not an approved therapeutic regimen, but is documented as a major drug-drug interaction of clinical significance[2][3].
