Overview

Nesiritide + milrinone is a combination of two intravenous agents used experimentally or off-label in the acute management of decompensated heart failure and for perioperative hemodynamic support, particularly after cardiac surgery. Nesiritide is a recombinant B-type natriuretic peptide that acts primarily via vasodilation, natriuresis, and suppression of the renin-angiotensin-aldosterone system, reducing preload and afterload. Milrinone is a small molecule phosphodiesterase III (PDE-III) inhibitor with positive inotropic and vasodilatory effects; it increases cardiac contractility and peripheral vasodilation by preventing cAMP breakdown in cardiac and vascular smooth muscle cells. The combination may theoretically provide complementary hemodynamic benefits (vasodilation, improved cardiac output) but clinical studies have not shown significant improvement in outcomes compared to individual use in perioperative settings[1][3][5][7].
