Overview

Neuroprotectin D1 (NPD1) + Resolvin D1 (RvD1) is a combination therapy composed of two specialized pro-resolving mediators (SPMs) derived from docosahexaenoic acid (DHA). This docosanoid combination is being investigated for the treatment of acute ischemic stroke, where it acts to resolve neuroinflammation and promote tissue repair. The mechanism involves the activation of specific G protein-coupled receptors (such as FPR2/ALX, GPR32, and GPR37) to antagonize pro-inflammatory signaling, reduce leukocyte infiltration, and enhance the clearance of apoptotic cells by macrophages. Preclinical studies in rat models of middle cerebral artery occlusion (MCAo) have demonstrated that the combination significantly reduces cortical and subcortical lesion volumes and provides sustained neurobehavioral recovery when administered within a 6-hour therapeutic window. The therapy also modulates the expression of homeostatic microglial and astrocyte genes to restore brain homeostasis after injury.
