Overview

Nicardipine + sodium nitroprusside is a combination of two intravenous antihypertensive agents used for the rapid control of severe hypertension, particularly in critical care and perioperative settings. Nicardipine is a dihydropyridine calcium channel blocker that inhibits the influx of extracellular calcium into vascular smooth muscle and myocardium, leading to vasodilation and decreased systemic vascular resistance. Sodium nitroprusside (Nipride) is a direct-acting vasodilator that releases nitric oxide to relax both arteriolar and venous smooth muscle, resulting in rapid reduction of blood pressure. This combination may be considered when monotherapy with either agent does not achieve adequate blood pressure control or when tailored hemodynamic effects are desired. Both drugs act via different mechanisms but share the goal of prompt blood pressure reduction; their use requires close monitoring due to potential side effects such as hypotension or cyanide toxicity (with prolonged nitroprusside use).
