nifedipine + enalapril

nifedipine + enalapril is a combination therapy being investigated by The Cleveland Clinic for the management of postpartum hypertension and preeclampsia. Nifedipine is a dihydropyridine calcium channel blocker that inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle, leading to vasodilation and decreased peripheral vascular resistance. Enalapril is a prodrug that is hydrolyzed to enalaprilat, a potent angiotensin-converting enzyme (ACE) inhibitor; it lowers blood pressure by reducing the production of angiotensin II and decreasing aldosterone secretion. This combination is being evaluated in the NEPHRON clinical trial to determine if it provides superior blood pressure control compared to the standard regimen of nifedipine and labetalol in the postpartum period.
