Nifedipine + pentoxifylline is a compounded combination drug, typically formulated as a topical cream or gel, designed to improve blood flow and alleviate symptoms associated with poor peripheral circulation. Nifedipine is a dihydropyridine calcium channel blocker that inhibits L-type voltage-gated calcium channels in vascular smooth muscle and myocardial cells, leading to vasodilation and reduced blood pressure. Pentoxifylline is a methylxanthine derivative that improves microcirculation by decreasing blood viscosity, increasing erythrocyte flexibility, reducing plasma fibrinogen levels, inhibiting neutrophil activation, and suppressing platelet aggregation; it also has anti-inflammatory effects. The combination acts synergistically to enhance peripheral perfusion and tissue oxygenation. This compounded medication is primarily used for conditions such as Raynaud’s phenomenon, intermittent claudication due to peripheral arterial disease (PAD), venous stasis ulcers, diabetic ulcers, chilblains (pernio), pressure ulcers, hypertension-related skin changes, and other disorders involving compromised microvascular circulation[1][3][4].
