NIM811 + peginterferon + ribavirin is an investigational combination therapy for chronic hepatitis C virus (HCV) infection, particularly genotype 1. NIM811 (SDZ 211-811) is a non-immunosuppressive cyclophilin inhibitor developed by Novartis. It is a small molecule cyclosporine derivative that targets host cyclophilins, specifically cyclophilin A (PPIA), which are cellular peptidyl-prolyl isomerases required for the formation of the HCV replication complex. Unlike cyclosporine A, NIM811 lacks calcineurin-mediated immunosuppressive activity. The combination includes peginterferon alpha (a pegylated cytokine) and ribavirin (a nucleoside analog), which were the standard-of-care backbone for HCV treatment. The triple combination was studied to enhance antiviral efficacy and suppress the emergence of drug resistance in HCV patients, especially those who had relapsed on previous interferon-based therapies.
