Nintedanib + azithromycin is a combination drug consisting of two active pharmaceutical ingredients with distinct mechanisms and indications. Nintedanib is a small molecule tyrosine kinase inhibitor that targets multiple receptor tyrosine kinases involved in fibrotic processes, including receptors for vascular endothelial growth factor (VEGF), platelet-derived growth factor (PDGF), and fibroblast growth factor (FGF). It acts by blocking downstream signaling pathways in fibroblasts, thereby exerting antifibrotic, anti-inflammatory, and anti-angiogenic effects. Nintedanib is primarily indicated for the treatment of idiopathic pulmonary fibrosis and other progressive fibrosing interstitial lung diseases to slow lung function decline. Azithromycin is a macrolide antibiotic belonging to the azalide subclass of macrolides. It inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, preventing transpeptidation during translation. Azithromycin has broad-spectrum antibacterial activity against various Gram-positive and Gram-negative bacteria as well as atypical pathogens such as Mycoplasma and Chlamydia species. Besides its antimicrobial effect, it also exhibits immunomodulatory properties by reducing pro-inflammatory cytokines and enhancing anti-inflammatory cytokines. Azithromycin is used widely for respiratory tract infections, skin infections, sexually transmitted infections, among others. The combination may be used clinically where both antifibrotic therapy with nintedanib and antibacterial/immunomodulatory therapy with azithromycin are indicated concurrently or synergistically beneficial.
