Overview

**Nirmatrelvir + ritonavir** is an oral, co-packaged antiviral combination marketed principally as **Paxlovid** for treatment of mild-to-moderate COVID-19 in adults at high risk of progression to severe disease, including hospitalization or death. Nirmatrelvir is a selective, reversible covalent inhibitor of the SARS-CoV-2 main protease, also called 3CL protease or Mpro, thereby preventing viral polyprotein processing required for replication. Ritonavir is included primarily as a potent CYP3A inhibitor that slows nirmatrelvir metabolism and increases its systemic exposure; this pharmacokinetic boosting function also creates clinically important drug-drug interaction risks. Paxlovid is supplied as co-packaged oral nirmatrelvir and ritonavir film-coated tablets and is generally administered for 5 days.
