Overview
Nitrous oxide (N2O), commonly known as laughing gas, is an inhalational anesthetic being investigated as a rapid-acting treatment for treatment-resistant depression (TRD). In the SMILE (Sustained Mood Improvement with Laughing Gas Exposure) trial led by Women's College Hospital, it is administered at a 50% concentration for one hour in combination with intravenous saline. Nitrous oxide acts primarily as an N-methyl-D-aspartate (NMDA) receptor antagonist, a mechanism shared with ketamine, which is thought to modulate glutamatergic neurotransmission to alleviate depressive symptoms. This specific combination and protocol are being evaluated for their ability to provide sustained mood improvement in patients who have failed multiple standard antidepressant therapies.
