The combination of **nivolumab** (an anti–PD-1 monoclonal antibody marketed as Opdivo) and **epacadostat** (a selective small molecule inhibitor of indoleamine 2,3-dioxygenase 1, IDO1) is a combination immunotherapy investigated for a range of advanced solid tumors and lymphomas. **Nivolumab** blocks PD-1, a negative regulator of T-cell activity, thereby enhancing anti-tumor immune responses. **Epacadostat** inhibits IDO1, an enzyme that mediates immune suppression by degrading tryptophan; by blocking IDO1, epacadostat may reduce immunosuppression in the tumor microenvironment, further augmenting T-cell activity. This combination is under clinical investigation, including in the ECHO-204 trial, primarily for melanoma, squamous cell carcinoma of the head and neck, non-small cell lung cancer, colorectal cancer, ovarian cancer, glioblastoma, and diffuse large B-cell lymphoma.
