Nivolumab + ipilimumab is a combination immunotherapy regimen being investigated by the Icahn School of Medicine at Mount Sinai for the treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL). This protocol, often referred to as 'immunotransplant,' combines dual checkpoint inhibition with adoptive T-cell transfer (ATCT). Nivolumab, a human IgG4 monoclonal antibody, targets the programmed death-1 (PD-1) receptor, while ipilimumab, a human IgG1 monoclonal antibody, targets cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4). By blocking these inhibitory pathways, the combination aims to enhance the anti-tumor activity of both endogenous and adoptively transferred T cells following lymphodepleting chemotherapy. This approach is specifically targeted at DLBCL patients who are ineligible for standard autologous stem cell transplant due to chemotherapy insensitivity or inability to tolerate high-dose regimens.
