Overview

**Nivolumab + ipilimumab + cobimetinib** is an investigational combination therapy consisting of three agents: nivolumab (an anti-PD-1 monoclonal antibody), ipilimumab (an anti-CTLA-4 monoclonal antibody), and cobimetinib (a small-molecule MEK inhibitor). Nivolumab and ipilimumab act as immune checkpoint inhibitors, blocking PD-1 and CTLA-4, respectively, to restore and enhance antitumor T-cell function and promote de novo antitumor T-cell responses. Cobimetinib targets the MEK pathway, a critical effector in RAS/RAF/MEK/ERK signaling, relevant for cancers with KRAS mutations such as pancreatic adenocarcinoma. This triplet regimen was studied in the CheckMate 032 trial for previously treated advanced/metastatic pancreatic adenocarcinoma, showing a manageable safety profile and modest activity in a small cohort. The combination leverages potential synergy between immune checkpoint blockade and MEK inhibition, aiming to increase tumor immunogenicity and T-cell infiltration[3].
