Overview

A four-drug combination regimen consisting of the immune checkpoint inhibitors nivolumab (a monoclonal antibody targeting PD-1) and ipilimumab (a monoclonal antibody targeting CTLA-4), combined with chemotherapy agents nab-paclitaxel (albumin-bound paclitaxel) and gemcitabine. This combination is under investigation primarily for pancreatic cancer, particularly advanced or metastatic pancreatic adenocarcinoma, in clinical trials. Nivolumab and ipilimumab work by blocking inhibitory pathways in T cells, thereby promoting immune-mediated antitumor activity. Nab-paclitaxel interferes with microtubule function, inhibiting cell division, while gemcitabine is a nucleoside analog that inhibits DNA synthesis. Together, the regimen aims to combine immunotherapeutic and cytotoxic mechanisms to improve tumor control[1][3][5][7].
