Overview

Nivolumab + ipilimumab + platinum-based chemotherapy is a combination regimen used primarily as a first-line treatment for metastatic or recurrent non-small cell lung cancer (NSCLC) without EGFR or ALK genomic aberrations. The regimen consists of two immune checkpoint inhibitors—nivolumab (a programmed death-1 [PD-1] inhibitor) and ipilimumab (a cytotoxic T-lymphocyte-associated antigen 4 [CTLA-4] inhibitor)—plus a platinum-based chemotherapy, commonly cisplatin or carboplatin (DNA crosslinking chemotherapeutics). Nivolumab enhances antitumor immune responses by blocking the PD-1 pathway, preventing T cell inactivation. Ipilimumab blocks CTLA-4, promoting T cell activation and proliferation. Platinum-based chemotherapy agents bind DNA, cause crosslinking, and induce immunogenic cell death, which may synergize with immunotherapy. Developed primarily by Bristol Myers Squibb, the regimen is recommended for use in adults, demonstrating superior overall survival and progression-free survival compared to chemotherapy alone in clinical trials (e.g., CHECKMATE-9LA)[1][3].
