nivolumab + relatlimab + gemcitabine + cisplatin is an investigational combination therapy being studied as a first-line treatment for patients with advanced biliary tract cancer. The regimen includes two monoclonal antibodies, nivolumab and relatlimab, which target the PD-1 and LAG-3 immune checkpoints respectively, to overcome T-cell exhaustion and enhance the anti-tumor immune response. These are combined with the standard chemotherapy doublet of gemcitabine (a nucleoside analog) and cisplatin (a platinum-based DNA alkylating agent). The National Health Research Institutes in Taiwan is evaluating this quadruple combination in the Phase II NOBLE trial to determine if the addition of dual checkpoint blockade improves objective response rates and survival outcomes compared to standard treatments in this patient population. Relatlimab restores the effector function of exhausted T cells by binding to the LAG-3 receptor, while nivolumab prevents immune evasion by blocking the PD-1 receptor.
