Overview

nivolumab + sunitinib + lirilumab is an experimental combination therapy regimen investigated for the treatment of advanced malignancies, particularly metastatic renal cell carcinoma (mRCC). This triplet regimen combines three distinct therapeutic mechanisms: **nivolumab**, a human monoclonal antibody that blocks the Programmed Death-1 (PD-1) receptor to enhance T-cell-mediated anti-tumor immunity; **lirilumab**, a human monoclonal antibody that targets Killer Cell Immunoglobulin-like Receptors (KIR) to prevent the inhibition of Natural Killer (NK) cells; and **sunitinib**, a small-molecule multi-kinase inhibitor that targets vascular endothelial growth factor receptors (VEGFR) and platelet-derived growth factor receptors (PDGFR) to inhibit angiogenesis and tumor cell proliferation. The combination aims to achieve a synergistic effect by simultaneously activating both adaptive and innate immune responses while concurrently disrupting the tumor's blood supply. Although individual components and doublet combinations have been extensively studied, clinical evaluation of this specific triplet has been limited due to observed toxicities in PD-1 and TKI combinations.
