Overview

Nivolumab + troriluzole is an investigational combination therapy being studied primarily in advanced solid tumors and melanoma. Nivolumab is a monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor, functioning as an immune checkpoint inhibitor to enhance anti-tumor immune responses. Troriluzole (BHV-4157, trigriluzole) is an oral prodrug of riluzole that inhibits glutamate release; it targets glutamatergic signaling implicated in tumor growth and resistance to immunotherapy. The rationale for this combination stems from preclinical evidence that glutamate signaling can activate MAPK and PI3K/AKT pathways in tumor cells, potentially contributing to resistance against immunotherapies like PD-1 inhibitors. Early-phase clinical trials have shown the combination to be safe and well-tolerated with some evidence of disease stabilization even in patients with prior PD-1/PD-L1 therapy failure[1][2][5]. The maximum tolerated dose of troriluzole was determined as 420 mg daily when combined with standard-dose nivolumab[5].
