Nivolumab + vopratelimab is an investigational combination therapy for advanced solid tumors. Nivolumab is a fully human IgG4 monoclonal antibody that targets the programmed cell death 1 (PD-1) receptor, blocking its interaction with PD-L1 and PD-L2 to enhance T-cell mediated antitumor responses. Vopratelimab is a monoclonal antibody agonist of the inducible costimulator (ICOS), designed to activate ICOS signaling on T cells and promote antitumor immunity. The combination aims to synergistically enhance immune activation by both relieving inhibitory signaling (via PD-1 blockade) and providing co-stimulatory signals (via ICOS agonism). Clinical trials have shown that this combination has a favorable safety profile but modest efficacy overall; however, clinical benefit appears enriched in patients with specific pharmacodynamic biomarkers such as emergence of ICOS-high CD4 T cells[1][2][4]. The primary developer of vopratelimab is Jounce Therapeutics.
