NKG2D CAR-NK is an investigational chimeric antigen receptor natural killer (CAR-NK) cell therapy developed by The Third Affiliated Hospital of Guangzhou Medical University for the treatment of metastatic colorectal cancer, specifically targeting liver metastases. The therapy utilizes NK cells engineered via mRNA electroporation to transiently express a chimeric receptor composed of the extracellular domain of the human NKG2D receptor fused to the DAP12 signaling domain. This construct allows the NK cells to recognize and eliminate tumor cells expressing NKG2D ligands, such as MICA, MICB, and UL16-binding proteins (ULBPs), which are frequently upregulated in colorectal cancer. The use of mRNA electroporation serves as a safety strategy by limiting the duration of CAR expression to prevent long-term off-target toxicities. In clinical studies, the cells are delivered through regional routes such as hepatic artery transfusion or direct percutaneous injection into liver lesions to maximize local effector concentration and therapeutic efficacy.
