Norepinephrine + Goal-Directed Fluid Therapy (GDFT) is a perioperative hemodynamic management protocol developed by RenJi Hospital for patients undergoing kidney transplantation. This therapeutic strategy combines a continuous intravenous infusion of norepinephrine (administered at a rate of 0.06 µg/kg/min) with a fluid administration regimen guided by stroke volume variation (SVV). Norepinephrine acts as a potent vasopressor, primarily targeting alpha-1 and beta-1 adrenergic receptors to maintain systemic vascular resistance and cardiac contractility. The GDFT component utilizes SVV as a dynamic indicator of fluid responsiveness to guide the administration of crystalloid boluses, ensuring optimal effective circulating blood volume and renal perfusion while preventing fluid overload. The protocol is specifically designed to reduce the incidence of delayed graft function (DGF), a common complication in renal transplantation characterized by the need for dialysis within the first week post-surgery.
