Overview

Norepinephrine + vasopressin is a combination therapy used primarily in the treatment of septic shock and other hypotensive states. This combination leverages complementary mechanisms to maintain blood pressure in critically ill patients. Norepinephrine is a sympathomimetic that acts on alpha-1 receptors to increase venous and arterial tone, while also affecting beta-1 receptors to increase preload and contractility. Vasopressin works through V1a receptors on vascular smooth muscle cells to cause vasoconstriction, particularly in the splanchnic circulation. The combination may provide synergistic effects, allowing for lower doses of each individual agent, potentially reducing side effects while maintaining adequate blood pressure. Studies suggest that combining these medications may improve renal function in septic shock patients compared to norepinephrine alone, with patients showing higher urine output and reduced serum creatinine levels.
