This entry refers to a clinical trial (NCT03115060) conducted at the Post Graduate Institute of Medical Education and Research (PGIMER), Chandigarh, comparing three standard intravenous crystalloid solutions: Normal Saline (0.9% NaCl), Plasmalyte A, and Ringer's Lactate. The study was a prospective, randomized, double-blind Phase 4 trial designed to evaluate the effects of these fluids on intraoperative acid-base balance and electrolyte levels in patients undergoing living donor kidney transplantation. Normal Saline is an isotonic solution containing sodium and chloride (154 mmol/L each). Plasmalyte A is a balanced crystalloid solution containing sodium, potassium, chloride, magnesium, acetate, and gluconate, formulated to closely mimic the electrolyte composition of human plasma. Ringer's Lactate is another balanced crystalloid containing sodium, chloride, potassium, calcium, and lactate. The trial specifically investigated the incidence of metabolic acidosis and hyperkalemia during the ischemia-reperfusion phase of renal transplantation, finding that balanced solutions (Plasmalyte and Ringer's Lactate) provided better pH and chloride stability compared to normal saline.
