The combination leverages the synergistic effects of two distinct pharmacological agents: pregabalin, a gabapentinoid, and nortriptyline, a tricyclic antidepressant (TCA). Pregabalin acts as a ligand for the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, which reduces the influx of calcium into presynaptic terminals and subsequently inhibits the release of excitatory neurotransmitters like glutamate and substance P. Nortriptyline functions by inhibiting the reuptake of norepinephrine and, to a lesser extent, serotonin, thereby enhancing the activity of descending inhibitory pain pathways.
