Overview

This is a fixed-dose combination of notoginseng total saponins (NS), the main bioactive components extracted from Panax notoginseng (Burk.) F. H. Chen, and safflower total flavonoids (SF), the primary active ingredients from Carthamus tinctorius L. The combination is widely used in traditional Chinese medicine for the prevention and treatment of cardiovascular diseases, particularly myocardial ischemia and infarction. Mechanistically, NS primarily exerts anti-inflammatory effects by reversing elevated proinflammatory metabolites in myocardial injury models, while SF mainly ameliorates oxidative stress by modulating antioxidant/prooxidative metabolites. When combined as CNS or NS-SF, these agents demonstrate synergistic cardioprotective effects greater than either component alone—regulating both metabolic targets to enhance efficacy against myocardial ischemia injury[1][2][4]. The mixture also modulates cytochrome P450 enzymes in vivo—inducing CYP1A2, 2B1, 2E1, 2C19 and 2D4 while inhibiting CYP3A1—which may contribute to its pharmacological synergy but also raises potential for herb-drug interactions[2]. Key constituents include notoginsenoside R1, ginsenosides Rg1/Re/Rb1/Rd from NS; hydroxysafflor yellow A and kaempferol-3-O-rutinoside from SF[4].
