Nous-209 + pembrolizumab is a combination investigational immunotherapeutic regimen composed of **Nous-209**, an off-the-shelf neoantigen cancer vaccine targeting 209 shared frameshift peptide neoantigens from mismatch repair deficient (dMMR) and microsatellite instability-high (MSI-H) tumors, administered together with **pembrolizumab** (Keytruda), a monoclonal antibody checkpoint inhibitor that targets the programmed death-1 (PD-1) receptor. Nous-209 consists of a genetic prime (GAd20-209-FSP) and boosting (MVA-209-FSP) vaccine components, both administered intramuscularly to induce anti-tumoral immune responses and promote T cell tumor infiltration. Pembrolizumab restores anti-tumor immunity by blocking the PD-1 receptor pathway. The combination is being evaluated primarily for first-line treatment of unresectable or metastatic dMMR/MSI-H colorectal cancer and other MSI-H/dMMR tumors. Clinical studies thus far indicate the combination is well-tolerated and elicits promising anti-tumor activity[1][2][3][5].
