This combination consists of three components: **NovoTTF-100L** (a Tumor Treating Fields device), **pemetrexed** (a multi-targeted antifolate chemotherapy), and **carboplatin** (a platinum-based chemotherapy). NovoTTF-100L is a wearable device that delivers low-intensity, alternating electric fields (Tumor Treating Fields, TTFields) to the thoracic region, disrupting mitotic spindle formation and inhibiting cancer cell division. Pemetrexed inhibits thymidylate synthase and other folate-dependent enzymes, interfering with DNA and RNA synthesis. Carboplatin forms DNA crosslinks, preventing DNA replication and transcription. The combination is approved for the first-line treatment of **unresectable, locally advanced or metastatic malignant pleural mesothelioma** and has been studied in clinical trials demonstrating improved survival relative to historical outcomes.
