**NY-ESO-1 + ISCOMATRIX** is a cancer vaccine that combines the recombinant protein antigen **NY-ESO-1** (New York Esophageal squamous cell carcinoma-1), a “cancer-testis” antigen frequently expressed in various cancers, with **ISCOMATRIX**, a saponin-based immunologic adjuvant designed to enhance both humoral and cellular immunity[1][3][5][8]. The vaccine is administered via intramuscular injection and was developed to induce broad-spectrum, antigen-specific CD4^+^ and CD8^+^ T cell responses as well as high-titer antibody responses, aiming for anti-tumor immunity in patients with NY-ESO-1-positive tumors[1][2][3][5][7][8]. NY-ESO-1 + ISCOMATRIX has primarily been studied in melanoma patients, both with resected tumors and advanced disease, in phase II clinical trials. Although it efficiently induces immune responses against NY-ESO-1, clinical endpoints such as relapse-free survival and overall survival have not shown significant improvement compared to control arms, possibly due to immune escape mechanisms in tumors[1][2][7]. ISCOMATRIX consists of saponins, cholesterol, and phospholipids, forming immune-stimulating complexes[1][3]. The vaccine has a favorable safety profile[1][2][5][6].
