NZV930 + NIR178 is a combination therapy studied for advanced malignancies. **NZV930** is a fully human monoclonal antibody that targets and antagonizes CD73, an ectoenzyme that regulates extracellular adenosine, aiming to enhance anti-tumor immune response by decreasing immunosuppressive adenosine levels. **NIR178** is a small molecule inhibitor of the adenosine A2A receptor (also known as ADORA2A), aiming to block adenosine-mediated immunosuppression within the tumor microenvironment. The combination is being studied to synergistically augment anti-cancer immune activity by dual inhibition of adenosine pathway signaling at both the production (CD73) and receptor (A2A) levels. Developed primarily by Novartis, the combination has been tested in patients with advanced cancers including non-small cell lung carcinoma, triple negative breast cancer, pancreatic ductal adenocarcinoma, renal cell carcinoma, ovarian cancer, microsatellite stable colorectal cancer, and metastatic castration-resistant prostate cancer. The combination was administered as intravenous NZV930 and oral NIR178 capsules; the recommended phase 2 dose was NZV930 600 mg IV every 2 weeks plus NIR178 up to 240 mg BID. Development for this combination was terminated early due to insufficient efficacy data, not for safety reasons[1][3][5].
