Obatoclax + carboplatin + etoposide is a combination of three antineoplastic agents investigated for use in cancer, specifically extensive-stage small cell lung cancer (ES-SCLC). **Obatoclax** is a small-molecule, pan-Bcl-2 family inhibitor that induces apoptosis in cancer cells by antagonizing anti-apoptotic members of the Bcl-2 family. **Carboplatin** is a platinum-based chemotherapeutic that crosslinks DNA, inhibiting DNA synthesis and function. **Etoposide** is a topoisomerase II inhibitor that prevents DNA re-ligation, leading to DNA strand breaks and cell death. This combination has been studied primarily in ES-SCLC, aiming to enhance apoptosis in cancer cells and improve clinical response beyond carboplatin plus etoposide alone. Early clinical results showed the triplet was generally well tolerated and transient central nervous system side effects with obatoclax infusion were manageable, but the addition of obatoclax did not significantly increase response or survival outcomes in phase II studies[1][3][7][9].
