Overview

Oblimersen + dacarbazine is a combination therapy investigated primarily for the treatment of advanced malignant melanoma. Oblimersen (also known as G3139) is an antisense oligonucleotide that targets the mRNA of B-cell lymphoma 2 protein (Bcl-2), an antiapoptotic protein often overexpressed in cancer cells, thereby promoting apoptosis and potentially overcoming chemotherapy resistance. Dacarbazine (DTIC) is a small molecule antineoplastic agent classified as an imidazole carboxamide derivative; it acts mainly through alkylation of nucleic acids, inhibiting DNA synthesis and cell proliferation. The combination was studied in phase III clinical trials to determine if targeting B-cell lymphoma 2 protein (Bcl-2) with oblimersen could enhance the efficacy of dacarbazine in patients with advanced melanoma. While the combination improved progression-free survival and response rates compared to dacarbazine alone, it did not achieve a statistically significant improvement in overall survival for all patients; however, certain subgroups (such as those without elevated baseline serum LDH) showed increased benefit[2][5][6].
