Octreotide-LAR + Lanreotide Autogel is a combination therapy involving two long-acting depot somatostatin analogues (SSAs) being evaluated for the treatment of advanced hepatocellular carcinoma (HCC). The combination is specifically studied in patients with liver cirrhosis whose tumors express somatostatin receptors (SSTRs). Octreotide-LAR (Sandostatin LAR) is a synthetic peptide analogue of somatostatin administered intramuscularly, while Lanreotide Autogel (Somatuline Depot) is a deep subcutaneous injection. Both drugs function as agonists of somatostatin receptor subtypes 2 and 5 (SSTR2 and SSTR5), leading to the inhibition of hormone secretion and tumor cell proliferation, as well as a reduction in circulating insulin-like growth factor-I (IGF-I) levels. This therapeutic approach is being investigated by the University of Naples Federico II in the SS-HCC pilot study.
