Overview
Octreotide + pantoprazole is a combination therapy being evaluated for the management of acute non-variceal gastrointestinal bleeding (NVGIB) in critically ill patients. Octreotide, a synthetic octapeptide analogue of somatostatin, functions as a potent inhibitor of various gastrointestinal hormones, including gastrin and secretin, and specifically reduces splanchnic blood flow. Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by inhibiting the H+/K+-ATPase enzyme system at the secretory surface of gastric parietal cells. This dual approach is intended to stabilize blood clots and reduce the risk of rebleeding by maintaining a neutral gastric pH and decreasing local blood flow. The combination is currently under investigation in a Phase 3 clinical trial sponsored by the National Hepatology & Tropical Medicine Research Institute.
