Overview

Odevixibat + maralixibat is a combination of two small molecule drugs that act as reversible inhibitors of the ileal bile acid transporter (IBAT), also known as the apical sodium-dependent bile acid transporter (ASBT). Both agents are approved individually for the treatment of cholestatic pruritus in pediatric patients with progressive familial intrahepatic cholestasis (PFIC) and Alagille syndrome (ALGS). By inhibiting IBAT in the terminal ileum, these drugs block reabsorption of bile acids from the intestine, leading to increased fecal excretion and reduced serum bile acid levels. This mechanism helps alleviate pruritus associated with cholestatic liver diseases. Odevixibat is marketed under the brand name Bylvay and developed by Albireo Pharma (now part of Ipsen), while maralixibat is marketed as Livmarli by Mirum Pharmaceuticals[1][3][4][5][6].
