olanzapine + fluoxetine

**Olanzapine + fluoxetine** is a fixed-dose oral combination of olanzapine, an atypical antipsychotic, and fluoxetine, a selective serotonin reuptake inhibitor. It was developed by Eli Lilly and marketed in the United States as **Symbyax** for acute depressive episodes associated with bipolar I disorder and for treatment-resistant depression in adults. Fluoxetine increases serotonergic signaling through serotonin-transporter inhibition, while olanzapine antagonizes dopamine and serotonin receptors and has additional histaminergic, muscarinic, and adrenergic receptor activity. Important safety liabilities include weight gain, increased appetite, dyslipidemia, hyperglycemia, sedation, anticholinergic effects, extrapyramidal symptoms, serotonin syndrome risk, antidepressant-associated suicidal thoughts and behaviors in younger patients, and increased mortality in elderly patients with dementia-related psychosis.
