Olanzapine + modafinil is a combination of two pharmaceutical drugs, each with distinct mechanisms of action. Olanzapine is an atypical antipsychotic primarily used to treat schizophrenia and bipolar disorder. It acts as an antagonist at multiple neuronal receptors, including dopamine D1-D4 receptors, serotonin 5HT2A/2C/3/6 receptors, alpha-1 adrenergic receptor, histamine H1 receptor, and several muscarinic receptors. Its antagonism at the dopamine D2 receptor in the mesolimbic pathway reduces positive symptoms of psychosis while its effect on serotonin 5HT2A helps mitigate negative symptoms[6]. Modafinil is a wakefulness-promoting agent approved for narcolepsy, obstructive sleep apnea-related excessive sleepiness, and shift work sleep disorder[1][5]. It acts mainly as a weak inhibitor of the dopamine transporter (DAT), increasing extracellular dopamine levels; it also inhibits noradrenaline reuptake in the ventrolateral preoptic nucleus (VLPO), has partial agonist activity at alpha-1B adrenergic receptors, activates glutamatergic circuits and inhibits GABAergic transmission[5][8]. Modafinil may be used adjunctively with olanzapine to counteract sedation or weight gain associated with antipsychotic therapy; however, evidence for long-term efficacy or safety in this combination remains limited[3][2].
