Olaptesed pegol + filgrastim is an experimental combination regimen combining olaptesed pegol, a pegylated L-oligoribonucleotide inhibitor of the chemokine CXCL12, with filgrastim, a recombinant human granulocyte colony-stimulating factor (G-CSF). Olaptesed pegol functions by blocking the interaction between CXCL12 and its receptors (CXCR4/CXCR7), thereby disrupting the chemokine-mediated retention of hematopoietic and malignant cells within the bone marrow microenvironment, with intent to sensitize tumor cells to chemotherapy or other interventions. Filgrastim stimulates the production and mobilization of neutrophils and hematopoietic progenitor cells, reducing neutropenia and potentially enhancing stem cell mobilization for transplantation. This combination is under clinical investigation for its effects on mobilization of hematopoietic stem cells and for potential therapeutic benefit in oncology indications.
