Olaptesed pegol (NOX-A12) is a PEGylated L-stereoisomer RNA aptamer that acts as an antagonist of the chemokine CXCL12 (also known as SDF-1). It disrupts the interaction between CXCL12 and its receptors, thereby modulating the tumor microenvironment to enhance immune cell infiltration and sensitize tumors to immunotherapy. Pembrolizumab is a monoclonal antibody targeting PD-1, blocking its interaction with PD-L1/PD-L2, thus promoting T-cell mediated anti-tumor responses. The combination of olaptesed pegol and pembrolizumab is being investigated in clinical trials for treatment-refractory solid tumors such as metastatic colorectal cancer and glioblastoma, aiming to synergistically improve anti-tumor immunity by both disrupting immune evasion mechanisms in the tumor microenvironment (via CXCL12 inhibition) and releasing T-cell inhibition (via PD-1 blockade)[5][6][7].
