Olaratumab + doxorubicin + ifosfamide is an investigational combination regimen for the treatment of advanced or metastatic soft tissue sarcoma. **Olaratumab** is a monoclonal antibody that selectively binds to and inhibits platelet-derived growth factor receptor alpha (PDGFRα), blocking PDGF-mediated signaling and subsequently tumor cell proliferation. **Doxorubicin** is an anthracycline chemotherapy agent that intercalates DNA and inhibits topoisomerase II, leading to DNA damage and apoptosis. **Ifosfamide** is an alkylating agent that causes cross-linking of DNA strands, disrupting DNA replication and inducing cell death. Early studies indicate that adding olaratumab to doxorubicin and ifosfamide is feasible and active, with a safety profile comparable to doxorubicin and ifosfamide without olaratumab[1][3][5].
