Olaratumab + pembrolizumab is an investigational combination therapy consisting of two monoclonal antibodies—olaratumab, an anti–platelet-derived growth factor receptor alpha (PDGFRα) antibody, and pembrolizumab, an anti–programmed cell death protein 1 (PD-1) antibody. Olaratumab inhibits tumor cell proliferation and signaling by antagonizing PDGFRα, which regulates cell growth and tumor stroma interactions. Pembrolizumab blocks the PD-1 checkpoint on immune cells, thereby unleashing anti-tumor immune responses. The rationale for their combination is to use olaratumab’s alteration of the tumor microenvironment and stroma to enhance the susceptibility of tumors to immune checkpoint blockade by pembrolizumab. This combination has been studied in patients with unresectable, locally advanced or metastatic soft-tissue sarcoma with disease progression on standard treatments. Early-phase clinical studies show manageable safety, tolerable adverse event profile, and evidence of durable disease control and partial responses in some sarcoma subtypes[3][5].
