olesoxime + riluzole

**Olesoxime + riluzole** is an oral combination therapy investigated for the treatment of amyotrophic lateral sclerosis (ALS). Olesoxime is a small molecule mitochondrial pore modulator developed by Trophos, designed to promote neuronal survival by interacting with mitochondrial membrane proteins (TSPO and VDAC), reducing mitochondrial permeability transition and oxidative stress, and promoting neurite outgrowth and nerve repair. Riluzole, the standard of care for ALS, is a neuroprotective small molecule that inhibits glutamate release, blocks voltage-gated sodium channels, and interferes with intracellular signaling downstream of excitatory amino acid receptors. Clinical trials of the combination in ALS did not show a significant benefit on overall survival compared to riluzole alone, but olesoxime was well tolerated when used together with riluzole.
