Overview

Olutasidenib is a selective, brain-penetrant, orally bioavailable inhibitor of mutant isocitrate dehydrogenase 1 (IDH1), developed for the treatment of cancers harboring IDH1 mutations. It works by blocking the activity of mutant IDH1 enzymes, which are implicated in tumorigenesis through production of the oncometabolite 2-hydroxyglutarate. Olutasidenib is approved for relapsed or refractory acute myeloid leukemia (AML) with susceptible IDH1 mutations and has demonstrated activity in gliomas with IDH1 mutations[4][6][7]. Temozolomide is an oral alkylating agent that damages DNA by methylation at several sites, leading to cell death; it is primarily used to treat glioblastoma multiforme and refractory anaplastic astrocytoma[1][2][8]. The combination of olutasidenib and temozolomide is being investigated as maintenance therapy after radiotherapy in pediatric and young adult patients with newly diagnosed high-grade glioma (HGG) harboring an IDH1 mutation[3][5].
