Overview
This therapy is a **combination regimen** being investigated for **platinum-resistant or platinum-refractory ovarian cancer**. It consists of: - **olvimulogene nanivacirepvec** (Olvi-Vec), an oncolytic vaccinia virus engineered to selectively infect and replicate in malignant cells, thereby lysing tumors and activating both innate and adaptive immune responses; - **platinum-based chemotherapy** (e.g., carboplatin or cisplatin), DNA-alkylating agents causing cell death by DNA cross-linking and immunogenic cell death; - **non-platinum chemotherapy** (e.g., gemcitabine, paclitaxel, docetaxel, or pegylated liposomal doxorubicin), agents with diverse cytotoxic mechanisms; - **immune checkpoint inhibitor** (e.g., bevacizumab, which inhibits VEGF-mediated angiogenesis, though it is not a classical checkpoint inhibitor, but sometimes included in immunochemotherapies). Olvi-Vec is administered intraperitoneally, followed by systemic chemotherapy. The synergy is hypothesized to arise from virus-mediated immune activation and re-sensitization of tumors to chemotherapy, including reversal of platinum resistance.
