Overview

Omalizumab + immunotherapy refers to the **combination therapy** of **omalizumab**, a recombinant humanized monoclonal IgG1 antibody targeting IgE, with **allergen immunotherapy** (AIT), typically subcutaneous or rush immunotherapy. Omalizumab binds to free IgE, blocking its interaction with the high-affinity IgE receptor (Fc-epsilon-RI) on mast cells, basophils, and dendritic cells, leading to downregulation of these receptors and reduced allergic inflammatory responses. Allergen immunotherapy involves controlled exposure to allergens to induce immune tolerance and reduce allergic symptoms. The combination is primarily used in patients with allergic diseases—most notably allergic asthma, allergic rhinitis, and food allergies—when AIT alone is not sufficiently effective or when AIT poses an elevated risk of severe allergic reactions. Clinical studies and meta-analyses have shown that omalizumab + immunotherapy significantly improves efficacy (higher rates of achieving target maintenance dose and sustained unresponsiveness) and safety (fewer severe systemic adverse events) compared to AIT alone[1][3][5][7][9].
