**Omaveloxolone + ipilimumab** is an investigational combination therapy for cancer, primarily studied in metastatic or unresectable melanoma. Omaveloxolone is a small molecule Nrf2 activator that inhibits myeloid-derived suppressor cells (MDSCs), restoring immune activity and suppressing tumor-induced immune evasion. Ipilimumab is a monoclonal antibody that acts as a checkpoint inhibitor by targeting cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), releasing inhibition of T-cell activation. The rationale for this combination is that omaveloxolone counters immune suppression in the tumor microenvironment, potentially overcoming resistance to checkpoint inhibition and enhancing antitumor immune responses. It is currently in clinical trials and has not been approved for this indication[1][5][7].
