Omega-3 fatty acids, primarily eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), are long-chain polyunsaturated fatty acids typically derived from fish oil. In the context of sepsis, they are utilized for their potent immunomodulatory and anti-inflammatory properties. These fatty acids function by competitively inhibiting the metabolism of arachidonic acid through the cyclooxygenase (COX) and lipoxygenase (LOX) pathways, thereby reducing the production of pro-inflammatory eicosanoids. Furthermore, they serve as precursors for specialized pro-resolving mediators (SPMs), such as resolvins and protectins, which actively promote the resolution of inflammation. Menoufia University conducted a randomized clinical trial (NCT04121520) to evaluate the efficacy of enteral omega-3 supplementation in reducing organ dysfunction, as measured by SOFA scores, and modulating inflammatory markers in critically ill septic patients.
