Omega-3 fatty acids, specifically eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), were investigated by Medical Center Alkmaar (Northwest Clinics Alkmaar) in the OLIVIER trial (Omega-3 Fatty Acids in Bariatric Gastric Bypass Surgery). This Phase 4 randomized controlled study evaluated the pre-operative use of approximately 2 grams per day of omega-3 supplementation in morbidly obese female patients undergoing laparoscopic Roux-en-Y gastric bypass (LRYGB) surgery. The primary objective was to reduce liver volume and visceral fat area to facilitate surgical access, serving as a more patient-friendly alternative to traditional low-calorie diets (LCD). Mechanistically, these fatty acids act as agonists for PPAR-alpha and GPR120 (FFAR4), modulating lipid metabolism and exerting anti-inflammatory effects. While the trial demonstrated significant reductions in liver volume, it did not show significant improvements in peri-operative erythrocyte function compared to a low-calorie diet.
